Conflict of Interest Acknowledgement & Disclosure

I acknowledge and confirm that I have carefully read and completed this Statement of Disclosure in connection with my employment by, or service as an officer or member of the Board of Directors of, the Fair Housing Center of Metropolitan Detroit (FHCMD). According to Board policy, this statement must be completed prior to each Annual Meeting of the Members of the FHCMD. 

 

DEFINITIONS

 

A related party transaction is any transaction, agreement, or other arrangement in which a related party has a financial interest in which the FHCMD would be a party. Related parties include: You, members of your family, including your spouse or domestic partner, your parents, siblings, children, grandchildren, or great grandchildren. Any entity in which you or members of your family have an ownership interest.

 

REQUIRED DISCLOSURES

 

I shall report to the Board of Directors:

 

1.   Any entity for which I, or my family, serve as an officer, director, trustee, member, partner, owner, or employee which is the subject of a fair housing complaint received by FHCMD or is a party involved in litigation with the FHCMD;

2.   Any transaction in which the FHCMD is a participant and in which I or a member of my family may have a conflicting interest, including related party transactions;

3.   All other facts or circumstances that might constitute a conflict of interest, including, but not limited to any employment, ownership interest, consulting, or agency which might be a conflict of interest.

4.  Any other matter that I need to disclose in accordance with the FHCMD Office Administration and Human Resource Manual & Code of Conduct (November 2023).
